Foundayo: The New GLP-1 Pill You Can Take Anytime — Prescription Guide
On April 1, 2026, the FDA approved Foundayo (orforglipron) — the first small-molecule oral GLP-1 receptor agonist for weight management. Unlike the Wegovy pill, Foundayo can be taken at any time of day, with or without food, and without water restrictions.
Here's everything you need to know about getting a Foundayo prescription.
What Makes Foundayo Different?
Foundayo's active ingredient, orforglipron, is a fundamentally different molecule from semaglutide. While semaglutide (Wegovy, Ozempic) is a peptide — a chain of amino acids that must be protected from stomach acid — orforglipron is a small molecule. It survives digestion intact, which is why you can take it without fasting or water restrictions.
This is a genuine convenience advantage. The Wegovy pill requires you to take it on an empty stomach with no more than 4 ounces of water, then wait 30 minutes before eating or drinking anything else. Foundayo has none of those restrictions.
Efficacy
In the Phase 3 trial (72 weeks, adults with obesity or overweight), Foundayo delivered average weight loss of approximately 7.5% to 11.2% depending on dose. By comparison, injectable Wegovy averages about 15% and injectable Zepbound about 20–21%. The Wegovy pill averages about 17% in those who stayed on treatment.
Foundayo is less potent than the injectables — but for patients who won't use needles, it's a real option that didn't exist before.
Dosing
Foundayo uses a gradual dose-escalation schedule to minimize GI side effects. You start low and increase over several weeks. If you miss 7 or more consecutive doses, you restart at a lower dose.
How to Get a Foundayo Prescription
- Check eligibility: BMI ≥ 30, or BMI ≥ 27 with at least one weight-related condition (hypertension, dyslipidemia, prediabetes, etc.)
- See your provider: Your PCP, endocrinologist, or a telehealth provider can prescribe it
- Route to LillyDirect: For the most reliable access and $149/mo self-pay pricing, ask your doctor to send the prescription through LillyDirect
- Insurance check: Some commercial plans are adding Foundayo coverage; check your formulary. It's also covered under the Medicare GLP-1 Bridge starting July 2026.
Who Should Consider Foundayo
- Patients who refuse injections
- Patients who struggle with the Wegovy pill's fasting requirements
- Patients transitioning from injectable GLP-1s to a maintenance oral regimen
- Patients starting GLP-1 therapy for the first time who prefer convenience
Who Should NOT Switch to Foundayo
If you're succeeding on Zepbound (tirzepatide), the data says stay on it. A clinical trial showed patients who switched from tirzepatide to Foundayo regained about 20% of their lost weight, compared to only 5% regain when switching from Wegovy injections. The reason is structural: Zepbound works on both GIP and GLP-1 receptors, while Foundayo targets GLP-1 alone.
Prefer Compounded Semaglutide Instead?
Foundayo is brand-name only — there's no compounded version. If you want compounded semaglutide at a lower price point, these providers offer legitimate options through licensed pharmacies:
