Getting a Wegovy Prescription for MASH: Fibrosis Tests and Coverage
Wegovy can be prescribed specifically for MASH, the inflammatory form of fatty liver disease. It received FDA accelerated approval on August 15, 2025, for adults with noncirrhotic MASH and moderate to advanced (F2 to F3) fibrosis. To get it covered for that use, your prescriber needs to document both the MASH diagnosis and the fibrosis stage, usually with noninvasive tests such as FibroScan rather than a biopsy. The prior authorization also needs to be filed under the liver diagnosis, not weight loss.
Key takeaways
- The MASH indication applies to Wegovy injection at 2.4 mg weekly. The Wegovy pill isn't approved for MASH.
- AASLD's November 2025 guidance says candidates can be identified with noninvasive tests. For example, a FibroScan stiffness of 8 to 15 kPa is consistent with F2 to F3 fibrosis.
- Cirrhosis has to be ruled out, because the approval covers noncirrhotic MASH only.
- Medicare's GLP-1 Bridge excludes people with fatty liver disease. Medicare.gov says their regular Part D plan might still cover a GLP-1 for that condition.
What the label actually approves
| Label element | What it says |
|---|---|
| Indication | Noncirrhotic MASH with moderate to advanced fibrosis (consistent with F2 to F3) in adults, with diet and physical activity |
| Approval type | Accelerated approval; continued approval may depend on a confirmatory trial |
| Form and dose | Injection, 2.4 mg once weekly maintenance dose |
| Not covered by this indication | Wegovy tablets; patients under 18; cirrhosis |
| BMI requirement | None stated for MASH; limited information below BMI 25 |

That last row matters for coverage. Unlike the weight-management indication, the MASH indication doesn't depend on a BMI cutoff. A person with MASH and F2 fibrosis who wouldn't qualify for Wegovy for weight loss may still match the liver indication.
The approval rests on the first part of the ESSENCE trial. In the week-72 analysis of 800 patients with biopsy-confirmed F2 or F3 fibrosis, published in the New England Journal of Medicine:
- Steatohepatitis resolved without worsening fibrosis in 62.9% of patients on semaglutide, compared with 34.3% on placebo.
- Fibrosis improved without worsening steatohepatitis in 36.8%, compared with 22.4%.
The trial's second part, which looks at liver outcomes such as cirrhosis, is expected around 2029 (Fierce Pharma).
Documenting the fibrosis stage: the usual path
On November 7, 2025, the American Association for the Study of Liver Diseases (AASLD) published updated guidance on semaglutide for MASH. According to HCPLive's summary, it says candidates can be identified with noninvasive tests rather than biopsy. Here's how that usually unfolds:
- A blood-based screen. Many clinicians start with FIB-4, a score calculated from age, AST, ALT, and platelet count. MDCalc notes that it is less reliable in people younger than 35 or older than 65.
- A second-line stiffness test. This is usually vibration-controlled transient elastography (VCTE, often called FibroScan). Magnetic resonance elastography (MRE) or the ELF blood test are alternatives.
- Ruling out cirrhosis. Results in a "gray zone" need extra work before treatment.
- Biopsy when needed. A biopsy isn't the default under the 2025 guidance, but a specialist may still recommend one if results conflict.
| Test | Range consistent with F2 to F3 (treat) | Gray zone (individualized) | Suggests cirrhosis (not approved) |
|---|---|---|---|
| VCTE (FibroScan) | 8 to 15 kPa | 15 to 20 kPa | Above 20 kPa |
| MRE | 3.1 to 4.4 kPa | 4.4 to 5.0 kPa | Above 5.0 kPa |
| ELF score | 9.2 to 10.5 | 10.5 to 11.3 | Above 11.3 |

For gray-zone results, the guidance suggests treating only after cirrhosis is excluded. That can be done with a second noninvasive test, or with imaging that shows no nodular liver surface or signs of portal hypertension. Ask your specialist which approach was used.
Who usually writes it
Any licensed prescriber can write Wegovy for MASH. In practice, the workup often runs through a hepatologist or gastroenterologist, because they order elastography and interpret conflicting results. Primary care doctors and endocrinologists can also prescribe it, especially when the test results are clear-cut. Weight-management telehealth visits are typically built around the obesity indication. If you suspect MASH, it's worth asking whether the clinician can order or review liver stiffness testing before you start.

Coverage: why the diagnosis on the request matters
Insurers can treat a weight-loss prescription and a liver-disease prescription for the same drug differently. Medicare is the clearest example. It keeps people with fatty liver disease out of its weight-loss Bridge program and points them to their regular Part D plan, which might cover the drug for that condition (Medicare.gov). Commercial plans vary, so your prescriber's prior authorization should lead with the liver diagnosis and the fibrosis evidence.
Diagnosis codes
In the ICD-10-CM code set, MASH is coded under K75.81, which is still titled "Nonalcoholic steatohepatitis (NASH)" (ICD10Data). Fibrosis has its own subcodes under K74.0, including K74.0A for moderate and K74.02 for advanced fibrosis (ICD10Data). Your prescriber's office chooses the codes. It's reasonable to ask which ones went on the request if a denial mentions "weight loss."
What a strong prior authorization packet includes
| Item | Why plans ask for it |
|---|---|
| MASH diagnosis with date | Matches the labeled indication |
| Liver stiffness or ELF result in the F2 to F3 range, with date | Documents moderate to advanced fibrosis |
| Evidence cirrhosis was excluded | The indication is noncirrhotic only |
| Other causes of liver disease considered | Shows the diagnosis is MASH and not another liver condition |
| Diet and activity plan | The label pairs treatment with diet and physical activity |
| Current medicines, including any GLP-1 | The label doesn't recommend combining Wegovy with other GLP-1 drugs |
Based on the indication and limitations in the Wegovy label; individual plan criteria vary, so ask for your plan's written policy.
Medicare
The Medicare GLP-1 Bridge excludes people with fatty liver disease. Medicare.gov says their Part D plan "might cover" GLP-1 drugs for that condition (Medicare.gov). The Bridge runs from July 1, 2026, through December 31, 2027. Check whether Wegovy is on your plan's formulary and what prior authorization it requires. Open Enrollment for 2027 coverage runs through December 7, 2026.
Commercial insurance and self-pay
If your commercial plan covers Wegovy, Novo Nordisk's Wegovy price guide says you may pay as little as $25, subject to a maximum savings of $100 a month, with government beneficiaries excluded. Without coverage, the same guide lists the self-pay price for most Wegovy pen doses at $349 a month as of October 2026. Under the self-pay terms shown on TrumpRx, cash purchases can't be counted toward your deductible or out-of-pocket limit.
Questions to bring to your appointment
- Has my fibrosis stage been measured, and with which test? Can I get a copy of the report?
- Do my results fall in the range consistent with F2 to F3, or in a gray zone that needs another test?
- Has cirrhosis been ruled out, and how?
- Will the prior authorization be filed for MASH rather than weight management? Which diagnosis codes will you use?
- If my plan denies it, will your office request a peer-to-peer review or file an appeal?
- Which test will we repeat to judge whether it's working, and when?
Asking these up front keeps the paperwork matched to the indication. If a denial cites a weight-loss exclusion, check whether the request went in under the wrong diagnosis.
After you start: reauthorization and monitoring
Many prior authorizations are time-limited, so plan to show progress at renewal. The AASLD guidance acknowledges that no noninvasive test reliably predicts biopsy response in an individual. It does list signals that suggest response at 72 weeks, such as a 30% or greater drop in VCTE liver stiffness or a 20% or greater drop in MRE stiffness (HCPLive). Keep copies of every elastography report.

Frequently asked questions
Is Wegovy FDA-approved for fatty liver disease?
Wegovy injection received accelerated approval on August 15, 2025, for adults with noncirrhotic MASH and moderate to advanced (F2 to F3) liver fibrosis, used with diet and physical activity. The Wegovy pill does not carry the MASH indication.
Do I need a liver biopsy to get Wegovy for MASH?
Not usually. AASLD's November 2025 guidance says candidates can be identified with noninvasive tests, such as a FibroScan liver stiffness of 8 to 15 kPa, though a specialist may still recommend biopsy if results conflict.
Do I need a certain BMI to get Wegovy for MASH?
The MASH indication doesn't set a BMI cutoff, though the label notes limited information in people with a BMI below 25. Coverage depends on documenting MASH and F2 to F3 fibrosis without cirrhosis.
Can I get Wegovy for MASH through the Medicare GLP-1 Bridge?
No. The Bridge excludes people with fatty liver disease. Medicare.gov says their Part D plan might cover a GLP-1 for that condition, so check your plan's formulary and prior authorization rules.
What dose of Wegovy is used for MASH?
The label's recommended maintenance dose for MASH is 2.4 mg injected once weekly, reached by gradual titration. Your prescriber sets the dose and titration schedule.
