Manufacturer Direct Channels: A Third Option

Between insurance-covered pharmacy and cash-pay telehealth sits a third channel that many patients don't know about: manufacturer direct. Both major GLP-1 manufacturers — Eli Lilly (Zepbound) and Novo Nordisk (Wegovy) — operate direct-to-patient pharmacy channels that offer their brand-name medications at reduced self-pay prices.

These channels serve patients who have a prescription but struggle with insurance coverage or high copays. They're not telehealth platforms — they don't prescribe. You bring the prescription; they fill it at a negotiated price.

LillyDirect (Zepbound)

LillyDirect connects patients with prescriptions to Lilly's pharmacy partners at self-pay pricing that's significantly below retail. The platform has offered Zepbound at reduced monthly rates for cash-pay patients, though specific pricing changes frequently. The service includes home delivery, typically with cold-chain shipping for injectables.

Who should use it: patients who have a Zepbound prescription from their doctor but whose insurance doesn't cover it, or whose copay is higher than LillyDirect's self-pay price. It's also useful for patients between insurance plans or during PA appeal periods.

NovoCare Pharmacy (Wegovy)

NovoCare is Novo Nordisk's patient support platform that includes pharmacy services, savings programs, and patient assistance. For eligible patients, NovoCare can significantly reduce the cost of Wegovy through manufacturer savings cards, patient assistance programs (for qualifying income levels), and pharmacy price negotiations.

NovoCare's savings card can reduce commercially insured patients' copays to as low as $0-$25 per fill for an initial period. The patient assistance program may provide Wegovy at no cost for patients who meet income eligibility criteria.

Manufacturer Channels vs. Telehealth: When to Use Which

FactorManufacturer DirectTelehealth Platform
PrescribingNo — bring your own RxYes — prescribers on staff

Monthly cost$300-500 (self-pay reduced)$149-399 (compounded)
FDA-approvedYesNo (compounded)
Ongoing careNo — you manage separatelyYes — prescriber included

The manufacturer channel makes the most sense when you value FDA-approved products and already have a prescriber managing your care. Telehealth makes sense when you want bundled prescribing + medication at the lowest cost, and the compounded vs. brand distinction is acceptable to you.
